SeniorBridge is offering PRN to Part-Time Care Management work with flexible hours, including days, evenings, and weekends. On-call coverage is minimal.

Responsibilities
Job Description
As a Home Care Manager you will direct care to clients and their families using a multidisciplinary approach. The RN Home Care Nurse position includes the following:
Conducting admission visits for new clients and their family
Conducting assessments to identify problems, eligibility for assistance and need for services
Ensuring in-home care delivery is in accordance with each client's contractual program requirements and is high quality and complies with company-adopted, evidence- based clinical practice guidelines
Developing a professional care plan from the assessment findings and in collaboration with the clients Physician to ensure the facilitation and provision of the best possible care
Thorough and timely documentation of initial and ongoing assessments and outcomes
Communicate explanation of the care plan and all subsequent cost of all services to client and or Families, Attorney or Guardians
Act as a liaison to families as well as with collateral clinicians, professionals & agencies.
Effectively communicate all findings, outcomes, and plans that relate to the provision of client care to the involved parties
Work effectively with all departments to ensure the best delivery of service
Participate in case conferences with other disciplines providing care
Participate in the on call activities as directed by supervisor
Observes infection control practices and uses standard precautions
Participates in quality management/performance improvement activities
Performs any other duties as requested by the company
Schedule: Ideally available 10-20 hours per week and participation in the on-call rotation. On-Call is Monday-Thursday 5PM-8:30AM and Friday 5:00PM-Monday 8:30AM.
Required Qualifications
Active RN license in the state of Florida with no restrictions
At least 2 years of experience of hospital or home care experience
Experience working with the adult population
Ability to travel to member's residence within 30 to 40 miles
Exceptional communication and interpersonal skills with the ability to quickly build rapport
Ability to work with minimal supervision within the role and scope
Ability to use a variety of electronic information applications/software programs including electronic medical records
Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel
Excellent keyboard and web navigation skills

This role is a part of Humana's Driver Safety program and therefore requires and individual to have a valid state driver's license, a clean driving record and proof of personal vehicle liability insurance with at least 100,000/300,000/100,00 limits.
This role is considered patient facing and is part of Humana At Home's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Associates working in the State of Florida will need ACHA Level II Background clearance
Preferred Qualifications
BSN
3-5 years of in home assessment and care coordination experience
Knowledge of community health and social service agencies and additional community resources
Experience with health promotion, coaching and wellness
Previous managed care experience
Bilingual - English, Spanish
Certification in Case Management
